Helpful tips for the Day of your Appointment
- Please Bring A CURRENT Photo ID and Valid Insurance card (If your insurance requires you to choose a Primary care Doctor) Make sure Trudell Doctor MD and Associates or One of our providers is on your card* we will not see you unless you are assigned to our office.
- You may print the New Patient paperwork and fill out prior to your visit to save time. Please Arrive a few minutes prior to your appointment time.
- Our office offers Telemed for Established patients, therefore if you have fever or Covid/Flu symptoms you may schedule a Telemed appointment ( these appointments typically have a 2 hr window)
- Call ahead if you are running late. We do allow a short grace period, but please be aware that if you arrive late, your wait may be longer. We will do all we can to accommodate your appointment and to minimize the need to reschedule.
- If you need to cancel or reschedule, we do ask that you give a 24 hour notice, so we are able to offer the appointment time to another patient.
- The office charges a $25 no-show fee.
